Key Requirements
Bachelor's degree in Education, Counseling, Social Work, or a related field.
Demonstrated experience working with adolescents, particularly in special education or transition services.
Ability to develop, implement, and monitor Individualized Transition Plans (ITPs).
Strong knowledge of state and federal regulations concerning student transition services (e.g., IDEA).
Proficiency in conducting age-appropriate transition assessments to determine student needs and interests.
Excellent communication skills for collaborating with students, parents, school staff, and external agencies.
Experience coordinating community-based instruction and vocational placement opportunities.
Ability to maintain accurate and confidential student records and documentation.
